The Forest Avenue Station has a single island platform that is canopied for most of its length, and before the station was renovated in 2013 had seen better days.
The single staircase that provides access to the station exit is at the extreme northern end of the platform. It leads down to a small station house beneath the elevated where Myrtle Avenue's unusual, elevated line that operates above a private ROW (not a city street) crosses a complex intersection at a jump in the street grid. Two staircases lead down to the street, at the SE and NW corners of the intersection of Putnam Avenue and Fairview Avenue. Forest Avenue is just east of these intersections, forming a small triangle with Putnam and Fairview Avenues that has some greenery.
