77 Street is a local stop on the Lexington Avenue Line in an area where the local tracks are above the express tracks meaning that from the side platforms of the station the express tracks are not visible but their rumble beneath can be heard.

Each platforms layout is simple with no free crossover or under, just fare control areas with turnstiles (and the uptown side now unstaffed) ten steps up straight back from each platform and exits to two street stairs on either side of the respective side of 77 Street (west side Downtown, East Side Uptown) at the intersection of Lexington Avenue. This gives the stop and unprecedented eight street stairs at the same intersection, four for each direction.

The stop was renovated in 2004 with the gold boarders on blue name tablets and a gold boarded trimline with a blue background with 77s in it restored. Beneath each name tablet Lenox Hill Hospital is tiled.
