Milton is a Mattapan High-Speed Line Station located in a small town center type area with brick buildings all around. The stop has the standard two tactile strip side platforms (inbound has a canopy outbound has no shelter but a bench, both have ADA ramps at their fronts) with the level crossing connecting the two. The stop has three primary means of access. The first is a gap in the fence on the outbound platform from the Neponset Trail that follows the tracks through the station; the next is a staircase down from Adams Street's overpass to the inbound platform. The third is from the small (41 spaces) station park & ride lot along the inbound platform.
